On March 1, 2019, at approximately 10:30 PM, workers were attempting to rig down the gull wings of a catwalk. While removing the pins that were holding the gull wings in position, a worker's middle fingers were in holes at the bottom of the gull wings. When the pins were removed, the gull wings swung together and amputated the tips of his middle fingers below the first knuckles. | |||||

At approximately 3:30 AM on March 5, 2015, an employee cleaning a pit was pushing mud toward a vacuum hose when he felt a pull in his groin area. A supervisor and employees aided the employee who exited the pit and, upon further examination, determined that a hernia had occurred. The employee was admitted to the hospital and required surgery. |
